Markdown + HTML readers: be more forgiving about unescaped &.
We are now more forgiving about parsing invalid HTML with
unescaped `&` as raw HTML. (Previously any unescaped `&`
would cause pandoc not to recognize the string as raw HTML.)
Closes #2410.
