Improvements to Mac OS X package build:
+ Add "pandoc" as bundle identifier to get rid of warning.
+ Remove old make_osx_pkg_dirs script, no longer needed because of
make target.
+ Add OSX-Welcome, with information about adding /usr/local/bin
to PATH.
+ Modify Makefile accordingly.
