Remove GFM modules; use CMarkGFM for both gfm and commonmark.
We no longer have a separate readGFM and writeGFM;
instead, we'll use readCommonMark and writeCommonMark
with githubExtensions.
It remains to implement these extensions conditionally.
Closes #3841.
