HTML reader: improve support for table headers, footer, attributes
- `<tfoot>` elements are no longer added to the table body but used as
table footer.
- Separate `<tbody>` elements are no longer combined into one.
- Attributes on `<thead>`, `<tbody>`, `<th>`/`<td>`, and `<tfoot>`
elements are preserved.
