Previously text that ended a div would be parsed as Plain
unless there was a blank line before the closing div tag.
Test case:
<div class="first">
This is a paragraph.
This is another paragraph.
</div>

Previously header ids were autogenerated by the writers.
Now they are generated (unless supplied explicitly) in the
markdown parser, if the `header_identifiers` extension is
selected.
In addition, the textile reader now supports id attributes on
headers.

AsiiDoc doesn't support them.

AsciiDoc does not seem to have consistent escaping rules.

The test output still contains some mistakes.

- proper escaping
- footnotes with blank lines - print separately at end?
currently they are just ignored.
- fix header (date gives weird result on pandoc README)
