Quite a few modules were missing copyright notices.
This commit adds copyright notices everywhere via haddock module
headers. The old license boilerplate comment is redundant with this and has
been removed.
Update copyright years to 2019.

Directives of this type without numeric inputs should not have a
`startFrom` attribute; with a blank value, the writers can produce
extra whitespace.

by the Markdown and LaTeX readers. There is no point to
this class, and it is not applied consistently by all readers.

RST does not allow nested emphasis, links, or other inline
constructs.
Closes #4581, double parsing of links with URLs as
link text. This supersedes the earlier fix for #4581
in 6419819b46c0d69c7024ba8aa4a6381cb311341c.
Fixes #4561, a bug parsing with URLs inside emphasis.
Closes #4792.

Previously pandoc would sometimes combine two line blocks separated by blanks, and ignore trailing blank lines within the line block.
Test is checked to be consisted with http://rst.ninjs.org/

...by parsing them as Span with "role" attributes.
This way they can be manipulated in the AST.
