* patches.lisp.pamphlet: Likewise.
* pathname.boot.pamphlet: Likewise.
* util.lisp.pamphlet: Likewise.
* bookvol5.pamphlet: Remove $spadroot throughout.
(initroot): Remove.
(reroot): Lose argument.
* Makefile.pamphlet (makeint.lisp): Depend on
sys-driver.$(FASLEXT). Explicitly load it.
Miscellaneous cleanup.
